An issued-with is two or more titles intended by the publisher to constitute a single publication. A bound-with is an item containing two or more titles bound together after publication by someone other than the publisher.
Clues that may indicate that an item is an issued with are:
If in doubt, consider the titles to be bound-withs.
General cataloging policy: Create a single record in The CAT for an issued-with; this record describes all of the titles in the item and provides author and/or title access to all the titles. Create a separate record in The CAT for each title in a bound-with.
A bound-with volume may contain items that were issued together. See the final example in An issued-with in a bound-with volume.
Search for copy for each title contained in the issued-with, but accept as matching copy only a single record that describes all of the titles. Use information from the separate records (name and title entries, notes, subject headings) in creating the issued-with record.
If the chief source for the item contains a collective title (a title that applies to all the works), record the collective title in field 245. If the separate titles as well as the collective title appear on the title page, give them in field 505 rather than field 245. [AACR2 1.1B10]

If there is no collective title, but there is a single title page that includes the titles of all the works, transcribe each of the titles from that title page in the order they appear, as instructed below. If there is no single title page, transcribe the title information from each of the separate title pages in the order they appear, as instructed below.
If all the works are by the same author, transcribe each title with its other title information. Precede the other title information by a space-colon-space ( : ). Separate each title statement by a space-semicolon-space ( ; ). The statement of responsibility (subfield c) follows all the titles. Place the subfield b following the title proper of the first work. If the works are by different authors, transcribe each title with its other title information and its statement of responsibility. Precede any other title information by a space-colon space ( : ). Precede any statement of responsibility by a space-slash-space ( / ). Place the subfield b before the other title information for the first work; if there is no other title information for the first work, there will be no subfield b in the field. Place the subfield c before the statement of responsibility for the first work or (if there is no statement of responsibility for the first work) before the title of the second work. If the titles of notes, appendices, and other accompanying works appear on the title page of the main work -- e.g., preceded by a phrase such as "To which are added, . . ." -- treat the item as an issued-with, even if the accompanying works are separately paged. Transcribe the title information pertaining to the accompanying works in the order in which the information appears on the title page, i.e., if the information appears before the statement of responsibility for the main work, transcribe it as other title information; if it appears after the statement of responsibility, transcribe it as a subsequent statement of responsibility. Accompanying work appears before statement of responsibility and other title is present: Tag Ind. Accompanying work appears after statement of responsibility: Tag Ind. Contents 245 10 Monsieur Bossu's treatise of the epick poem ... /|cdone into English from the French, with a new original preface upon the same subject, by W.J. ; to which are added, An essay upon Satyr, by Monsieur d'Acier ; and A treatise upon pastorals, by Monsieur Fontanelle.
If a single author is responsible for all the works, make the main entry (100) under the heading for that author.
If there are only two works, check the authority file to see whether a uniform title for that work has been established. If so, give that uniform title in field 240. If no uniform title has been established, omit field 240. Make an author/title added entry (700|a/|t) for the second work. Again, check the authority file to see whether a uniform title has been established. If so, give the uniform title in 700|t; if not, give the title proper of the second work in 700|t. [AACR2 25.7A]

If there are more than two works, make a 240 with the uniform title, "Selections."
If the works are by two or more authors, make the title the main entry. [AACR2 21.7B-C] Make author/title added entries (700|a/|t) for each work if there are no more than three; otherwise, do not make author/title added entries (unless requested by Rare Books). Check the authority file to see whether uniform titles have been established for any of the works; if so, give the uniform title in 700|t; if not, give the title proper in 700|t. [LCRI 21.7B]

A bound-with volume may contain items that were issued together. In such a case, create a single record for the items issued together, and add a 591 field as instructed in the bound-withs documentation.

The libretto and Gatty's essay were issued together; both are bound with a copy of the score.
